Multiple different endpoints can point to the same server node (e.g. 127.0.0.1
and 127.0.0.2).
This is likely a misconfiguration, but we should handle it gracefully.
* Log a warning
* Do not overwrite existing nodeChannelsByName entry
* Ensure proper cleanup
