Configuration now comes from {{clusterstate.json}}.
A plugin for a realistic placement use case has been added
({{SamplePluginAffinityReplicaPlacement}}) taking into account replica
distribution over availability zones in addition to number of cores and disk
space (and no two replicas of same shard on same node).
The code is largely (very largely) untested and unrun at this stage.
