I'm not aware of any Lucene-specific issue here. We see this mostly in unit tests, but it has also been replicated with production data. Although one can speculate this might be more of an issue in Lucene because of its segmented multi-graph structure; I'm not sure since I don't have all that much experience of HNSW in other settings.
