Pages that don't have an icon are generated by a plugin. The help-mojo is kind 
of tricky, its Java sourcefile is generated based on [a Java Sourcefile 
template|https://github.com/apache/maven-plugin-tools/blob/master/maven-plugin-tools-generators/src/main/resources/help-class-source.vm]
 and will be created when building any maven-plugin.

That also makes it hard to answer "all the current documentation". Most have a 
clear origin, but not all. First step would be to look in the page source. If 
that's not clear enough, just ask.
