Just to help you understand the problem: there are two different APIs to use 
iterators in UIMA. Here the helper class makes use of both. This is generally a 
bad idea because there's no guarantee the iterator won't break. Here however 
it's a straight up error in integrity checking: it's not even a compatibility 
issue between the two APIs.
