The writer document was provided to me privately so I didn't want to attach it here, and the problem seems clearly to be in the embedded svm file. Exporting the graphic using AOO 4.0.1 and replacing the embedded svm in the document file (using zip) fixes the document for AOO 4.1.1. So the problem seems to have nothing to do with the Writer document.
