It is interesting that opening the CSV in Excel 2016 produces a title row (of
too many columns) but only column A has the additional rows.

On inspection of the CSV, I notice that the columns are only comma-separated in
the first (title) row.  These commas are actually part of the titles and not
intended as separators, but Excel expects commas by default, it seems.

To import into OpenOffice Calc, I need to specify tab separation in the Import
filter.  When I do that I get a similar result to oooforum, except since "." is
the decimal separator on my system, the values with "." in column E are
correctly recognized as numbers.

I also have no problem opening this CSV in Apache OpenOffice 4.1.2 Calc.  This
test was on Windows 10 x64 Pro.

We need someone to try this on Mac OS X 10.
