Summary: Create Grafana dashboard for Ozone SCM safemode rules and
exit

In larger clusters, safemode exit can takes tens of minutes. We should be able
to track this over time in one place with a Grafana dashboard. This dashboard
should have one chart for each rule with two lines: One for the target value
and one for the actual value. It should also display a binary value as 1 if SCM
is in safemode, and 0 if it is out.
