[~szetszwo] Thanks for reviewing the patch! v3 patch addresses your comments. I
have also changed GrpcLogAppender#pendingRequests data structure to map in the
revised patch.
I have also made a change in the append entry response observer OnSuccess call.
If an appendEntry response times out, the reply is ignored and as a result the
follower's match index is not updated. Therefore I update match index if
heartbeat reply shows an extra entry in the follower.
