The problem is that the Struts2 doubleselect tag doesn't outputting the 
MULTIPLE attribute on the second select list when multiple="false" and 
doubleMultiple="true".  So fix this by adding the following to doubleselect.ftl 
under the simple theme and it's working:

 multiple="multiple"<#rt/>
</#if>
