I stumbled up on this, more or less by mistake. In my setup, if I enable parent 
proxy, then the synthetic health checks can trigger this. It'd only trigger 
once per day in my test, when the entry expires out of HostDB, but it pretty 
consistently reproduces if I clear HostDB first. All I did was enable the DNS 
thread, and parent proxying (the details there probably don't matter, as long 
as you have a least one rule in parent.config), and then I did
