This is a Reader problem, not an iText problem.
There is a newsgroup for such problems:

If you don't find a Reader that can print your A0 diveded
on A3-pages, you will have to split your page programatically. 

One possibility I can think of, is to put the content of
your page into a template (2 possibilities: or you write
the content directly to a PdfTemplate, or you parse your
existing PDF document with the PdfReader-class in which
case you'll get a PdfImportedPage).
Once you have your PdfTemplate or PdfImportedPage,
you can put the large page on different smaller pages
using different offsets.
