Use standard tools to create the PDF, with form fields marking the location of the modifiable areas. Then remove the form fields and save their location (and size, fontname, fontsize, etc.) to an XML file. Finally, merge the "personalized" data with iText and XSL.
Link points to example that does all this. Personalized data can be text, images or barcodes. The production side can easily be rewritten to use iText, but I don't think iText can extract all the necessary info necessary for the pre-production side.
