Thanks to all for the suggestions.

I look forward to looking at the PDFStamper API.  So it seems that best
approach is using pdf form fields.

Is Acrobat much easier to use in this regard?  I'm trying to gauge whether
it is worth the purchase.  Or will something like PDFTypewriter do?
