Graphics2D.create() creates a copy of the Graphics2D at the time is done. Scalings and translations that you do in this copy has no effect on the original. I'm shure this is the problem. A copy is created and the background is drawn in it, then the pictures in the original. With iText what you get is the pictures with the background on top.
No, the pictures are drawn correctly in the correct "layer" - but they have the wrong opacity set.
I just changed the /ExtGState /ca value and they appeared correctly in Acrobat.
