I have generated a table of contents in pdf, and it appears at the end
of the document.  Every page but the first one has a header, and a
footer with the page number (by design).   I then call
pdfWriter.reorderPages to move the TOC to the second page.  That is when
I run into a couple of issues that I don't know how to get around.

Issue #1: After reordering the pages, page 1 still does not have a
header or footer (as expected), page 2 has a header and a footer (as
expected), but page 3 does not have a header or a footer (incorrect).

Page reordering doesn't change content.


Issues #2:  Is there any way to change the header and footer for a given
page after it has already been added to the document.  After moving the
pages, although the TOC is on page 2, it still shows the original page
number where it was added.

Page reordering is a kludge. If you want it well done you'll need a two pass
approach, the first pass to generate a dummy pdf to know where things are
and the second pass to generate the final pdf.
