I have very successfully created a 22,000+ page (40 MB) document. We
build it and save it to the server file system then create a link to it.
Works very well.

You're sending docs to a browser and the creation of these docs takes
more than the timeout.
There are different ways to solve this problem.
This is one of them:
http://itextdocs.lowagie.com/tutorial/general/webapp/index.html#timeout
Another way to do it, would be to keep the connection between the
browser and server open.
But the last time I wrote such an example was over 6 years ago.
