I've got a basic idea of how it works so, with your hints, I should be able
to sort it out... although at the moment I think "pretty easy" is a fairly
relative term ;-).

         You need to first read the PDF Reference to understand how 
Object Data (aka User Properties) exists inside a PDF (hint: it's 
structure & tagging).

         Second, look at the sections of the iText sources concerned 
with the implementation of Marked Content and Structure.

         From there, it's pretty easy...
