The basic problem is that the generation of the table of contents is fully 
handled by Word and I haven't found a way to influence that yet. Some of the 
problems will be solved by switching to the RTF stylesheets that are under 
development, but that will take some more time. Since I'm just finishing up 
my master thesis, I won't be able to look into this in detail in the near 
future.
