You are confusing the word 'form' as is 'sheet of paper that can be
filled in manually'
with the technical term AcroForm which is an electronic form in PDF with
a set of
field annotations that mark specific areas on specific pages.
The PDF you have sent in attachment doesn't contain this AcroForm structure.
