insertPage is quick, but when you add a PdfImportedPage
all interactive features (for instance the form) are lost.
A PdfImportedPage is like a photocopy on paper.
Links, attachments, fields,... are not copied.

What you need to do is to read the form from PdfReader
and add the annotations one by one to page 1 with method
PdfStamper.addAnnotation(annot, 1);

The most difficult part will probably be to reconstruct
the fields based on the data retrieved with getAcroForm
or getAcroFields.

I see that you don't mind digging into specs; you may want
to have a look at the class AcroFields.java.
