Coords are relative to the origin (bottom left) of the MediaBox.

Conceptually, yes. It's not really 72dpi, but it's easier
to think of it that way.
The way I teach this to folks is that "if you mentally work
out all your coordinates in 72dpi, then everything 'just works'".

The size of the MediaBox is NOT effected by CTM's - it's fixed!
However, the contents of the page can be transformed all it
wants. But it will still be on an A4 page.
Do remember, however, that the MediaBox is the PHYSICAL SIZE
of the page - NOT the VISIBLE SIZE of the page...that's controlled by
the CropBox.
