Unfortunately the documentation isn't quite up to date.

Don't use RtfTOCEntry and your own styles if all you want to do is create a 
hierarchy of chapters. Simply use the RtfParagraphStyle.STYLE_HEADING_N to 
mark the chapter headings and they will automatically be added to the toc.

The RtfTOCEntry should only be used if you are adding toc entries that are not 
chapter headings.
