Of course you can.

At GovCamp I demonstrated this example:
http://itext.ugent.be/itext-in-action/examples/chapter17/FoobarCourses.java

The Servlet reads an existing PDF:
http://itext.ugent.be/itext-in-action/examples/chapter14/results/course_catalog_events.pdf
it extracts the bookmarks and generates an HTML:
http://itext.ugent.be:8080/itext-in-action/FoobarCourses
You can select some titles and the servlet will read the PDF again,
and create a new PDF with only the pages you selected.
