The behaviour was caused by a bug, for which I have committed a fix to the CVS. The reason was that the colours in the header were not being registered internally, before the final colour lists were written to the RTF document. Thus when you used the colours in the document as well, it worked, but not if they were never used at any other point. This should now work.
