I'm not sure how you define a line. Suppose you have: Quick brown fox jumps over the lazy dog. What is the first line according to you? Is it the first part of the sentence just before the newline? "Quick brown fox jumps over"? Or do you define line as the first complete sentence: "Quick brown fox jumps over the lazy dog"? If it's the former, I would use PdfContentByte and moveTo, lineTo, stroke based on the page size, page margins and leading. If it's the latter, I would probably put the sentence inside a Chunk, mark it with a generic tag, and wrap the Chunk in a Paragraph.
