I'm forwarding this to the mailing list. That's a better place
to ask questions: I'm just one person, currently in the hospital
with my son who has Cancer; the list has about 1,500 subscribers
so the chance to get an answer is much higher.

Signing a document multiple times involves that you have revisions.
That's inherent to the concept of signing PDFs (see the PDF Reference).
Each document can have one (and only one) author signature and
multiple recipient signatures. That's how it works in PDF.
You can have the PDF signed simultaneously by multiple people,
and each time a recipient signs, it has to be done in append mode
otherwise the previous signatures would be invalidated (for very
good reasons: by signing a document, you change it).
