I just did a little experiment to try to isolate the problem.

1. Opened enabled_form_prefilled.pdf by clicking on the link in the example, 
thereby opening it in Windows Internet Explorer.
2. Filled in 01 July 2008 as date and did "Save As" to Desktop.
3. Double-clicked on it to launch it in Adobe Reader 8.1.2.
4. Saw same message you saw,and that Bruno had cited in the osurce code for the 
example.

Now, I repeated the above steps, except in step 1, I right-clicked on the link, 
saving the Reader-Enabled PDF to my Desktop. Voila! Not broken when I added to 
it and saved it.

I have the same Windows XP SP2 and Adobe Reader 8.1.2 as you have.

I'm tentatively hypothesizing that there's an anomaly in the IE browser's 
integration with Adobe Reader.

Comments? Leonard? Bruno?
