I hear what you are saying about Adobe text being fixed/positional.  
But I still find it stunning that I CAN do this:

"Hello Mr. John Doe" (using an AcroForm variable)

But I Can't do this:

"Hello Mr. John Doe, we hope you are doing well"

I know Chapter 14 well but this does not provide a solution to allow a  
user to visually create a template.

Your statement that my request is impossible to fulfill could be true  
with regards to using a PDF document as a template. However, my desire  
to make this work is not unreasonable. Take, for instance, a PDF  
template that is provided in the LiveCycle Designer. A timesheet for  
instance. I can take this template and, using iText, generate 1000's  
of timesheets with dynamic data. This all works fine until someone  
asks me to add "Thank you XYZ Company For Your Business".

What a joke!

That's because what you're asking for is IMPOSSIBLE!

With PDF every character is put on an absolute position.
There is no "reflow".

You are confusing PDF with a Word Processing format.
XFA comes close to what you need, but the best way to achieve your goal
is to make your PDF as described in chapter 14 of the book "iText in
Action".
