You need to start by reading the Acrobat JavaScript documentation which will give you all the information about how to write the necessary scripts to associated with the buttons on the page.
I would recommend creating working buttons manually in Adobe Acrobat Professional first - and then once you have a working scenario, copy the scripts over to iText for automated building.
