you can look at the SimpleBookmark class:
http://www.1t3xt.info/api/com/lowagie/text/pdf/SimpleBookmark.html
1. create the reader and the stamper
2. get the bookmarks via SimpleBookmark.getBookmark(reader) static method. It
returns a list of HashMap objects: see the javadocs for more information
3. manipulate the list of HashMap objects by adding your new bookmark items
4. finally use myPdfStamper.setOutlines( myListOfBookmarks ) to save the
updated bookmarks to the resulting pdf.
