Your original file doesn't have the appearances generated and insted uses the
flag /NeedAppearances true to hint the viewer that it must generate the
appearances. The output of PdfCopyFields doesn't set this flag as it assumes
that the input PDFs are in a proper format (there are other nasty side effects
with this flag, in other words, don't use it). To fix it reopen the output PDF
and regenerate the appearances for each field with
AcroFields.regenerateField().
