I thought Word could create form fields... it's trivially easy to set
field values in iText.  I'm not sure how they'd survive the PDF
conversion process however.

You might also look into OpenOffice.org.  They have a PDF conversion
function that's quite good.  If they have an SDK that'll let you edit
the document, you're golden.
