Why are you talking about addNamedDestinations()? Your original question reads as: "I want to create an outline tree for an existing PDF." OK, there's an example on how to do that in the book. Shouldn't be any problem. But then you suddenly talk about named destinations. Are these two separate questions? Or did you take the wrong turn after your initial question. Maybe this is the problem: you assume that the person answering the question is familiar with the history of the question. That's not true. Questions are answered and the context is forgotten because of all of the other work. A subsequent questions is answered without knowing about the contex.
On another subject: it looks as if your not the only one not getting all the mails that are sent to the list. As I explained earlier, I consult the mailing list archives using NNTP (add news.gmane.org to a news account in your favorite mail client and pick gmane.comp.java.lib.itext.general), and I don't find all the messages I expected. I don't know what is causing this.
