It seems that 'padding-left' was fixed in 5.0.6 :

Maybe this is a similar issue. In the (old) version of iText I'm using, I
can't get margin-left to work either. I tried changing the markup to use
padding-left and that didn't work either. Upgrading to iText 5 is impossible
(that is, a lot of work), so it might be worth just sticking some spaces or
a tab at the beginning of the text (although that wouldn't indent the whole
paragraph, which might be useless).
