(why Adobe didn't include a Length attribute in inline images is just beyond
me)

Anyway, I submitted a code change a few weeks ago (rev 4799) that adds
handling for simple indexed color palettes in inline images.  Definitely try
the very latest code on your file.  If that still doesn't work, provide a
sample PDF that demonstrates the issue and I can take a look at what
characteristic of the inline image is causing the problem.
