That's correct.
Your initial question was about text and images (Image XObjects). Making 
sure you don't crop out annotations is fairly simple. However: if you 
want to avoid cropping out graphics state objects such as lines and 
curves, you should expect the development time needed to meet your 
requirement to increase exponentially.
