The PDF was created by software that doesn't respect the PDF specification.

(title "Fixing a broken form").

Also note that you're using a version of iText that is no longer supported.
If you ever get an audit, there will also be questions about the 
ownership of the code.
Please upgrade to iText 5 or higher.
