The text parser of iText, combined with LocationTextExtractionStrategy allows you to filter text extraction to a rectangular region. Grab the book, it's worth having and gives examples of how to do this (I can't recall if it has LocationTextExtractionStrategy in it, but I think it does - even if it doesn't, extending the regular text extraction example to do location filtering is straightforward once you understand how iText works).
