At first sight, I see two strange constructions in you code:1. you're creating a "poor man's table". That is: a table where you expect the font to be monospaced and you create columns by adding spaces. This is strange. One would expect you to use PdfPTable, or at least separator chunks. 2. instead of adding one Link annotation, you're adding two overlapping annotations. You should either use setAnchor() or setAction(). Not both.
Can you tell us which version of iText you're using? This looks like a bug that was solved a long time ago.
