Can you tell us which image you’re using and how you’ve installed the ICO HomeScreen? There is a zypper pattern you can use as there is more than one package needed, see https://wiki.tizen.org/wiki/IVI/Tizen-IVI_3.0-M2-March2014#Install_ICO_Homescreen.
The file that’s missing below (ivi-controller.so) is provided by the genivi-shell package which should be installed if you’re using the pattern.
