The correct directory is /lib/modules/`uname -r`/ivtv/ How about providing output of the 0.4.0 make? I had a problem with debian (something in the kernel headers i think) where the the modules were installed the /lib/modules/2.6.12/ivtv/ instead of /lib/modules/2.6.12-2-686/ivtv/ , this might be the cause here too. Look for the location of files called ivtv.ko and this will confirm.
