OK,

I think I have covered most of the suggestions posted in this thread:

1)  Renamed all instances of tuner.ko, msp3400.ko, and tda9887.ko to prevent these kernel version from being loaded.  This appears to have made no difference one way or the other.  I tried renaming each module one at a time, and making the corresponding alias entry in modprobe, but to no avail.
2)  Upgraded to ivtv driver 0.4.0.
3)  Updated my firmware to version 0x02050032.  This got rid of the warning I previously had. 
4)  Removed all instructions from modprobe.conf except the ones that appear to be necessary, as shown below.  After taking the actions of step 1) above, it became necessary to add the ivtv aliases or the modules could not be loaded.
5) Installed the mkinitrd changes and recreated my boot image.  Can anyone explain why this is necessary for the PVR-500?  Based on Jarod's site, it appears this change is necessary for the PVR-350 since it has on-board TV-out and piping X over that requires some modules be made available in the boot image?

After doing all this, I am still unable to get anything but static on /dev/video0.  There are 2 odd messages in the dmesg output below that I am unable to solve.

I'm really starting to wonder if there is some sort of hardware failure going on here.  My next step is to move the PVR-500 to my windows machine to check it out for some sort of hardware issue...unless someone has a better suggestion :(
