I did make changes to the filenames and some of the code to make it more
"inline" with the naming scheme of the rest of the drivers in the xorg
tree (call it ivtv_drv instead of ivtvdev).

Any chance on calling it ivtvdev_drv? I made a gentoo ebuild using the same plan of making it match the rest of xorg, so I took cues from the fbdev package (which names it ivtvdev_drv). That way xorg for all distibutions / versions will still just say ivtvdev and not ivtv. Alternatively, the decision can be made to change over to ivtv_drv as a name (and change all the distributions to match)

I posted links to that driver on this wiki, as well as instructions on installing it. Somebody else had written redhat install directions, but it may be good to add the updated directions for the new version, as well.
