I have the exact same problem you have (with the same card/tuner combination). 
Last week I decided to give kernel 2.4 a try instead of the 2.6 I was running 
and it just worked the first time I tried. It even kept working when I 
upgraded the i2c or bttv drivers for the 2.4 kernel. 

I used the same ivtv drivers for both the 2.4 and 2.6 kernels, so I think the 
problem is not with the ivtv drivers. The new i2c drivers on 2.4 didn't make 
a difference, so I don't think those are the problem either. That leaves the 
bttv drivers, but I couldn't find anything in the source of the tuner module 
which could break things.

As I prefer/need a 2.6 kernel this is not a solution I can use, but at least I 
got something out of the card.
