also running Gentoo; to get the 350 detected as card 0, swap the cards
from slot.
This is my /etc/modules.d/ivtv file for when I was running a PVR-350 +
150 (struggling right now to get a 2nd 150 running as well):

I autoload i2c_dev, lirc_dev, ivtv and lirc_i2c at boot (don't know if
they are all needed but it worked like that and haven't bothered
checking into that :-) ).

I also get those X-errors but they shouldn't cause you problems. I
think it's the ivtv-fb driver that isn't loaded correctly. Do you get
horizontal bars on the screen when the driver loads?
