The problem, that I just finally solved today due to the info above, is that for some reason, the default frame buffer is going to the main vid. card on your machine. By doing this, at least for me, it forced the card to take FB0, and the PVR-350 to FB1. Here is the relavent quote from the link above: "Modify the /boot/grub/grub.conf file. Remove the kernel option rhgb and add vga=792. Reboot This forces the console framebuffer to /dev/fb0 and my 350 to /dev/fb1. The vga option can be any valid value. I just picked the 1024x786 option." Once you do this, you'll have to change the values in your xorg.conf file. For the PVR-350, change the FB to 'FB1' (if that's what it's picking up...), and I also had to change preceding device on that from 'fbdev' to 'ivtv' I saw this as a bug in the driver code while looking throug more issues, and I now finally have X over the PVR-350 TV Out. I haven't tried a dual-head yet, and I still need to work on the resolution of the TV (720x480 works except that the screen is way to big for the tv, and I can't figure out how to change the overscan settings for X over the FB). Last but not least, there is one line I had to place into my rc.local due to having a newer PVR-350 card. My IVTV/LIRC would not start just via modprobe, so I had to call them all from rc.local.
