It doesn't happen on my IE (I'm using IE 8).
I have the option "File -> Duplicate current tab" that opens the same
HTML page in a new tab, but the option "File -> New Tab" opens a page
with the URL "about:Tabs", asking me what I wish to do (like re-open a
previous page, and so on). I think the URL "about:Tabs" can be changed
somewhere, so to display by default your preferred page instead.
I cannot imagine a cookie being the cause of the behavior you're
describing. Maybe there is some setting saying "always duplicate current
tab" or something like this.
