What you're hearing is the message that OE throws on the screen during the receiving or sending of mail. Sometimes, it doesn't clear itself immediately. There might be an error message there--use the JAWS Cursor to read the whole window. Do you have OE configured properly to retrieve your mail when it's opened? You should never have to press anything to have your mail retrieved.
As to your hearing "top" and "bottom," this isn't actually a loss of focus and aside from doing a JAWS repair, I can't suggest a possible solution.
