Almost forgot, have you actually confirmed with sighted assistance whether the start menu is actually opening or not, and Jaws just isn't reading the highlighted item?
Try changing your say all setting to all, or highlighted by doing the keystroke of insert+S, and see what is actually happening when you press the start menu.
